Please Note:

1.) Sending your creative art and design files with this form may take a long time to complete (several minutes to several hours if you have dial up Internet and have an extremely large file in excess of several MB) and will depend on the speed of your Internet connection as well as file size(s).

3.) Please make sure that all fonts and graphics are included.
